Acne Urticata

Acne urticata is a skin disorder characterized by the appearance of small itching papules particularly on the face, neck, upper arm, and upper trunk. Incidence is particularly seen in middle-aged women. A histological examination can help in confirmation of the condition. This condition can be very well treated with topical and oral medications.

Causes and risk factors

The exact mechanism and cause about acne urticata is still not clear. Although studies reveal that the condition is more commonly seen in women who undergo severe stress, but there still exists a debate whether the rash is triggered due to stress or the appearance of rash causes stress. It is said to be an immune reaction to the hair follicle.

 

Clinical presentation:

The most common site of affection is the face, neck, upper trunk, and upper arms. Eruption is seen in the form of small eruptions which are filled with fluid (papules), vesicular eruptions can also be seen. The affected area becomes reddened. Along with this, there is severe itching. Itching can aggravate the complaints causing a picking sensation. Slight soreness and burning can also be felt; severe itching can cause oozing of fluids from the bumps. This can lead to formation of crusty lesions. Itching can not only hamper the cosmetic appearance, but also lead to irritability and social embarrassment.

 

Investigations:

Diagnosis is done on the basis of the symptoms narrated by the patient and the physical examination carried out by the dermatologist. History of the patient is taken into consideration to elicit any contributing or maintain factors. Routine blood test, blood culture, or testing of the discharge along with histological examination is often diagnostic.

 

Treatment:

The dermatologist can recommend certain oral antibiotic medications. Application of topical antibiotics can also be advised. In severe cases, topical steroids are suggested. Administration of anti-inflammatory drug is other choice of medication. Phototherapy use has given positive results in most of the cases; however, it is contraindicated in cases where the eruptions are seen on sun exposed areas.

Complications:

As the complaints resolve, hypopigmentation at the affected area is seen.
