Delayed Ejaculation

Ejaculation means ejection of semen from the male reproductive organ (penis) which is essential for the conception. Delayed ejaculation is a sexual dysfunction in men characterized by prolong period of sexual stimulation to attend orgasm and eject semen during intercourse or by manual stimulation.

Causes and risk factors

Both somatic and psychological causes contribute to the causation of delayed ejaculation. As the age increases the sexual stimulation decreases leading to decreases sensitivity of o penis. Depression, anxiety, stress and disappointment in sexual relations are the major contributing factors. Medications that are used for treatment of depression, or high blood pressure can also lead to delayed ejaculation. It can also be an adverse effect of substance abuse and excessive intake of alcohol. Surgeries on the male reproductive system and trauma to the external genital organs also contribute to the causation. Delayed ejaculation can also be caused due to diseases like thyroid diseases, various neuropathies, certain infection, congenital defects in the male reproductive system etc. Diabetes is one of the common causes.

Symptoms 

The normal tine taken for ejaculation during sex is on the peak of attending orgasm by male partner. This may be anywhere from 3 to 10 min. Any abnormal delay or ejaculation over a period of 30-40min or failure to ejaculate is delayed ejaculation.

The patient often complains of delayed ejaculation. They need at least 30minutes or more while in some cases the man cannot reach orgasm. The complaints can either be temporary or permanent. It can also lead to psychological disturbances in both the partners.

Investigations

The diagnosis is done on the basis of the symptoms narrated by the patient. The physical examination is conducted to rule out congenital defect. Delayed ejaculation is diagnosed if the complaints persist for atleast3 quarters of sexual activity for a period of atleast6 months, when there is marked delay in ejaculation, the complaints cause distress in interpersonal relation and when the other underlying causes have been ruled out. Certain other sets of investigations can also be advised if there persist some underlying cause.

Treatment  

Treatment comprises of finding out the underlying cause. Appropriate treatment measures are then adopted. Medications to relief the complaints are administered. Counseling and psychotherapy are also effective. Visit to sex therapist by both the partners and sex therapy helps in overcoming the complaints.

Other modes of treatment:

The other modes of treatment can also be effective in treating the complaints. Homoeopathy is a science which deals with individualization considers a person in a holistic way. This science can be helpful in combating the symptoms. Similarly the ayurvedic system of medicine which uses herbal medicines and synthetic derivates are also found to be effective in combating the sufferings.
