Heck Disease

Heck disease is a rare oral disorder characterized by affection of the oral mucosa caused due to human papillomavirus (HPV). The incidence is commonly seen in children and in young adults. This condition is also known as focal epithelial hyperplasia.

Causes and risk factors

Heck disease is caused due to human papillomavirus. Studies have shown that the subtype 13 or 32 is responsible for this condition. This condition also seems to have a familial predilection. Poor hygienic condition adds up to the causation. People with weak immune system and those suffering from HIV infection are more prone to this condition.

 

Clinical presentation:

In most of the cases, the patient remains asymptomatic while those suffering from complaints present with appearance of small eruptions affecting the oral mucosa. The inner surface of the cheeks, lips, and tongue are commonly affected. Soft, smooth, and flat eruptions are seen. Their size is 1 cm or less. Multiple lesions are formed. They have a pale appearance while the rest of the mucosa shows whitish or reddish appearance. This all gives the mucosa a cobblestone appearance. The affected area is rough. The patient experiences soreness or slight burning on eating.

 

Investigations:

Diagnosis is done on the basis of the symptoms narrated by the patient and the physical examination carried out by the dermatologist. Routine blood test along with cultures is done. PCR analysis along with histological tests are done. If required, tissue biopsy with scraping can also be done to detect HPV subtype 13 or 32.

 

Treatment:

In case if the patient remains asymptomatic, no medical intervention is needed. Where necessary, the treatment plan consists of administration of medication. Oral antibiotics are started. Topical ointments and soothing agents are prescribed. If required, carbon dioxide laser or diathermy is done. Excisional biopsy is done in severe cases.

 

Other modes of treatment:
