Hemophthalmos

Hemophthalmos refers to bleeding that occurs within the eyeball. Depending upon the site of hemorrhage, hemophthalmos is known by several different names. Bleeding in the anterior chamber is known as hyphema, whereas in the vitreous is known as vitreous hemorrhage. Bleeding under the conjunctiva, retina, or macula is known as subconjunctival, subretinal, and subvitreous hemorrhage respectively.

Causes and risk factors

Hemophthalmos may occur as a result of physical trauma or due to underlying diseases such as hemophilia or diabetic retinopathy. The use of certain drugs such as anticoagulants can also lead to intraocular hemorrhage.

Clinical presentation

The symptoms show variation depending upon the site and severity of hemorrhage. Blurring of vision is a commonly occurring symptom. The patient experiences floaters and may be photophobic. Very rarely will the patient experience pain in the eye.

Investigations

Following an initial eye exam, an ultrasonography of the retina is performed in order to detect the severity of the hemorrhage.

Treatment

The treatment will be determined by the cause of hemophthalmos. Surgical intervention is necessary in most cases.

Complications

Blindness can occur as a result of severe hemophthalmos. Another known complication is glaucoma.
