Causes and risk factors
The disease occurs due to degeneration of spinous process and interspinous soft tissue. There is enlargement of the posterior spinous projections of the lumbar spine. It can be caused also due to repetitive stress and trauma.
Clinical presentation
Patient presents with gradually progressing low back pain. The pain is worse on extension of the spine and is relieved on flexion [bending]. The patient cannot bear contact with the affected part and is unwilling to bend.
Investigations
Medical history by the patient and clinical examination by the doctor helps in diagnosis. X-ray of spine is done. Articulation of spinous process, ‘kissing spine’ is diagnostic. MRI scan is recommended.
Treatment
Treatment consists of conservative management such as administration of analgesics and muscle relaxants. Physiotherapy helps in managing the condition. Nerve block injections may be required. Intralesional steroid injection, bursal excision, and decompressive laminectomy in severe conditions will also help in managing kissing spine.
