Causes and risk factors:
Nicotine stomatitis is seen in people who are addicted to smoking. The heat produced by the smoke of the cigarette causes the lesion. It is especially seen in people who smoke pipe or reverse cigarette smoking. Drinking of alcohol, hot drinks, or other irritants also predispose to this condition.
Clinical presentations:
Most of the time the patient remains asymptomatic, while some patients are not aware of the complaints. The mucosa of the hard palate along with soft palate is affected. Initially the mucosa becomes inflamed and appears red in color. Soreness is present. Burning pain is felt. The pain can be aggravated by eating sour, hot, and spicy food. Over time the area becomes white and gradually fissures develop. The mucosa has a cobblestone appearance.
Diagnosis and investigations:
Diagnosis is usually done on the basis of the symptoms narrated by the patient and a local examination carried out by the doctor. There is presence of history of smoking. Cessation of complaints on stopping of smoking confirms the diagnosis. However, even if complaints persist, a 5-mm punch biopsy is done.
Treatment:
Cessation of smoking is the main line of treatment. Smoking cessation program plays an effective role. The patient should be counseled for the same. Behavioral therapy also plays an important role.
Other modes of treatment:
The other modes of treatment can also be effective in treating the pain. Homoeopathy is a science which deals with individualization and considers a person in a holistic way. This science can be helpful in combating the symptoms. The Ayurvedic system of medicine which uses herbs and synthetic derivates can also be beneficial in combating the complaints.
