Brodies abscess

Brodie abscess is a type of osteomyelitis (infection of bone) characterized by formation of abscess with central accumulation of pus or jelly like substance surrounded by a zone of sclerosis. It is commonly seen in patients between the age group of 11-20 years. High incidence is seen in males as compared to females.

Causes and risk factors

Infection is one of the major causative factors. The infection from different parts of the body can reach the bones. A person’s immune system plays a major role in this. People with weak immune system are more prone towards developing this abscess.

 

Clinical presentation:

The lesions have been classified by Ross and Cole as aggressive types and another as cavities in epiphysis and metaphysis. Among the various bones of the body, the lower extremity is the most common bone of affection with higher chances of tibial affection. The upper end is particularly more commonly affected. Pain is the characteristic feature seen, which is aggravated by movement and is more common at night. Thickening of the bone occurs and local examination reveals tenderness. The overlying skin is warm to touch. In some cases over a period of time, muscle wasting can be seen.

 

Investigations:

Diagnosis is done on the basis of the symptoms narrated by the patient and the physical examination carried out by the doctor. X-ray of the affected joint is diagnostic.

 

Treatment:

Surgical evacuation and curettage is the main line of treatment. Antibiotics are simultaneously administered. In cases of large cavities, filling with cancellous bone chips is done.
